Description |
Maintain updated client records with plans, notes, appropriate forms, or related information. Advise clients or community groups on issues related to improving general health, such as diet or exercise. Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community. Assist clients with the intake process. Assess client’s immediate needs. Offer clients support, advocacy, referrals, and information about community resources. Provide clients with a shelter assessment when requested. Determine appropriateness of client placement in shelter and contact backup for final decision. Overall direct service client support.
